この度、Doctor’s Dilemmaに日本代表として出場し、ニューオリンズで世界の若手医師たちと知識を競い合ってきました。英語の早押し形式という非常に緊張感のある舞台で、結果は惜しくも初戦敗退となりましたが、一問一答に全力で挑んだ時間は、とても刺激的で、かけがえのない経験となりました。

世界の優秀な仲間たちと、医学という共通言語を通じて正々堂々と戦えたことは、何ものにも代えがたい財産です。自分の力不足を痛感する場面が多々ありましたが、それ以上に、医師という仕事の奥深さや尊さ、そして学び続けることの底知れない楽しさを、改めて実感しました。学会全体も、内科のあらゆる分野を網羅した充実のセッションばかりで、多くの学びを得ることができました。またニューオリンズは、歴史と芸術、音楽の息づく街であり、ジャズバーやミュージアムなど街そのものにも魅了されました。わずか4日間の滞在ではありましたが、心身ともに実り豊かな時間を過ごすことができました。

この出会いと挑戦の記憶を心の灯として、患者さんに寄り添う診療を大切にしながら、理想を追い求める姿勢を決して忘れず、歩んでいきたいと思います。

I had the great honor of representing Japan in the Doctor’s Dilemma at the American College of Physicians, held in New Orleans. There, I had the opportunity to compete with brilliant young physicians from around the world in a fast-paced, high-pressure quiz entirely in English. Although we were unfortunately eliminated in the first round, every moment spent answering each question with full concentration was deeply stimulating and truly unforgettable.

What left the greatest impression on me was the chance to engage in fair and spirited competition with peers from across the globe, united by the common language of medicine. While there were many moments in which I became acutely aware of my own limitations, the experience also reminded me of the profound depth and dignity of being a physician, and the endless joy that comes from continuing to learn throughout one’s career.

Beyond the competition, the conference itself was filled with enriching sessions that covered nearly every field of internal medicine. I gained valuable insights that I am confident will shape my future clinical practice. The city of New Orleans, with its rich culture, vibrant jazz bars, and world-class museums, added another layer of inspiration to the experience. Though my stay lasted only four days, it was a time full of intellectual and emotional fulfillment.

I will carry the memory of this encounter and challenge as a guiding light in my journey ahead—remaining close to my patients, practicing with sincerity, and never losing sight of the ideals I strive to uphold as a physician.

今回Doctor’s Dilemma に参加し，英語で医学を学ぶことや刺激を受け続けることの重要性を感じました。
最も海外との差を感じたのはリスニングの瞬発力でした。Dilemmaでは出題者によって問題が読み上げられた後，スクリーンに英文の問題が表示されその後早押しクイズ勝負となります。アメリカのレジデントは読み上げられた時点ですでに早押しの用意をしていますが，私は読み上げられただけでは理解できず，問題文を文字で確認する分遅れます。

MKSAPなどの問題を解くだけでなく，英語でのアウトプット，ポッドキャストなどでの情報収集を学習に取り入れて対策していきたいと感じました。
また，決勝進出するレベルのレジデントの知識水準の高さにも驚きました。決勝では日本語ですらわからないと感じる問題が多く，それをミスなく回答していく様は圧巻でした。自分もさらに研鑽を積まなければならないと感じ，とても良い刺激を受けたと思います。

今回はDoctor’s Dilemmaに参加することができ貴重な経験を積むことが出来たと思います。一緒に参加してくれた同僚の宮崎先生や，日本チームとして壇上で戦った瀧宮先生，仲良先生をはじめACP日本支部の方々にもサポートいただきました。大変感謝しております。来年に向けて日々の臨床能力向上と知識の拡充に励んでいきたいと思います。

Participating in Doctor’s Dilemma this year made me acutely aware of the importance of continually engaging with medical education in English and maintaining motivation through stimulating experiences. The most significant gap I noticed ompared to international colleagues was my ability to rapidly comprehend spoken English. In Doctor’s Dilemma, questions are first read aloud by the moderator and then displayed on a screen, triggering a quick-response quiz competition. American esidents
were prepared to buzz in immediately after hearing the question, whereas I found myself delayed because I needed to isually confirm the written question to fully understand it.
To bridge this gap, I recognize the need to go beyond merely solving MKSAP-style problems. I intend to incorporate active English communication and listening practices, such as podcasts, into my learning routine. Additionally, I was impressed by the exceptional level of knowledge demonstrated by residents who advanced to the final rounds. The finals
featured numerous challenging questions, many of which I felt I would struggle to answer even in Japanese, yet the inalists answered them flawlessly. Witnessing this high standard inspired me greatly, underscoring the necessity for continued self-improvement.

この度は大変光栄なことに、アメリカのルイジアナ州ニューオリンズで開催されたInternal Medicine Meeting 2025のDoctor’s Dilemmaに日本チームの一員として出場させていただきました。今回の渡航に関して多大なるご支援を賜りましたACP日本支部の皆様、新年度の忙しい時期に快く送り出してくださった獨協医科大学病院総合診療科の皆様に心より感謝申し上げます。

Doctor’s Dilemmaはレジデントを対象とした医学知識を競うコンペティション企画で、もともとは所属元の上級医に勧められて参加した前年度の国内大会が始まりでした。内容としてはクイズ大会に近く、当初は単にレクリエーションと認識して参加しておりましたが、その過程でこれは極めて有効な学習の機会であることに気づいていきました。

第一に、日本・アメリカいずれの大会でも非常に問題の質が高く、大会に参加するだけでも最新のエビデンスに基づいた医学知識をインプットする機会となります。特に競技形式ですので、正解したときの嬉しさ、間違えたときの悔しさというエピソード記憶に基づいて入力された内容はなかなか忘れません。

次に、チーム戦であるため大会までの間にチームや所属元の仲間と切磋琢磨できる点があります。実際、今回の国際大会に向けてはMKSAP 20を仲間とともに1ヶ月程度で解ききり、最終的にはほとんど正答できる状態で臨むことができました。この学習速度は自分ひとりではなし得なかった成果だと思っています。

そして最後に、そのようにして参加させていただいた国際学会は、Doctor’s Dilemmaだけでなく各分野最前線のセクションや、学会終了後のエキスパートの方々との懇親会など、単に知識の習得に留まらない非常に貴重な経験となりました。レジデントにとっては学会へエントリーするきっかけとしても大変有用な企画でした。

改めてこのような貴重な機会をいただきましたことに厚く御礼申し上げます。ここで得た経験を糧に日々の臨床でも研鑽を積んでいきたいと考えています。

I was deeply honored to participate in the Doctor’s Dilemma at the Internal Medicine Meeting 2025, held in New Orleans, Louisiana, as a member of the Japan team. I would like to express my heartfelt gratitude to the ACP Japan Chapter for their generous support in making this trip possible, and to the Department of Diagnostic and Generalist Medicine at Dokkyo Medical University Hospital for kindly allowing me to attend during the busy start of the new academic year.

Doctor’s Dilemma is a medical knowledge competition designed for residents. My journey began with a recommendation from my senior physician to participate in the national preliminary round last year. The format is similar to a quiz bowl, and while I initially viewed it as a recreational activity, I soon realized how effective it was as a learning opportunity.

First and foremost, the quality of the questions—both in the Japanese and U.S. competitions—was exceptionally high. Simply participating in the event provided an excellent opportunity to absorb up-to-date, evidence-based medical knowledge. Because of the competitive format, the emotional highs of answering correctly and the frustration of incorrect responses created strong episodic memories, making the content easier to retain.

Secondly, as a team competition, the preparation process allowed us to challenge and motivate each other. In fact, leading up to the international round, our team worked together to complete all of MKSAP 20 in about a month. By the time of the competition, we were able to answer most questions correctly—a level of preparation I could not have achieved alone.

Finally, attending the international conference was an invaluable experience far beyond the competition itself. In addition to Doctor’s Dilemma, I had the chance to attend cutting-edge sessions in various fields and engage with experts during social events after the conference. It proved to be a meaningful gateway into the world of international conferences for residents like me.

I would once again like to extend my sincere appreciation for this extraordinary opportunity. I am committed to applying what I have learned through this experience to my daily clinical practice and continuing to grow as a physician.

2025年4月にニューオリンズで開催されたIMM 2025 の参加報告

東京ベイ•浦安市川医療センター/伊東市民病院　内科専攻医　佐藤駿一

2025年4月10日から12日、アメリカ・ニューオリンズで開催された米国内科学会（ACP）年次総会2025に参加し、ポスター発表を行いました。

今回の参加は、米国内科学会日本支部総会2024で発表したIgA血管炎の症例報告により黒川賞を受賞し、Chapter Winnerに選出いただいたことがきっかけです。支部からの温かいご支援により、初めて海外の学会に参加する貴重な機会をいただき、心より感謝しております。

会場となったErnest N. Morial Convention Centerは規模も活気も圧倒的で、世界各国から集まった医師や医学生たちの熱意に刺激を受けました。
ポスター発表では、IgA血管炎の診断・治療経過について報告し、米国の先生方から多くの質問や意見をいただく中で、異なる医療アプローチを肌で感じることができました。

また、ポスターセッションの合間には、近くで発表していたアメリカのレジデントたちと交流し、互いの研修環境やキャリア観について語り合う機会にも恵まれました。この経験は、自分にとって非常に大きな励みとなりました。

今回得た学びを今後の臨床・研究活動に活かし、さらに成長していきたいと思います。貴重な機会をいただいたすべての皆様に、心より御礼申し上げます。

Report on Attending ACP 2025 in New Orleans

From April 10 to 12, 2025, I went to New Orleans, USA, to join a big medical meeting called ACP 2025. I gave a poster presentation there.

I joined because I won the Kurokawa Award after sharing a report about a disease called IgA vasculitis at the Japan Chapter meeting of ACP in 2024. Thanks to warm support from the chapter, I was able to join an international meeting for the first time. I am very thankful.

The place of the meeting, Ernest N. Morial Convention Center, was very big and full of energy. Doctors and medical students came from many countries. I was very excited and learned a lot from them.

In my poster presentation, I talked about how we found and treated the IgA vasculitis case. Many doctors from the U.S. asked me good questions. I could learn how medical care is different in other countries.

During free time in the poster session, I also talked with American resident doctors near my poster. We shared stories about our training and future dreams. That was a very happy and helpful time for me.

I want to use what I learned here in my future study and work. I thank everyone who gave me this great chance.

この度、2025年度Rising Star ScholarshipのAwardeesに選出頂くという光栄に預かり、Generalistとしてのキャリアを考える上での貴重な機会として、New Orleansで開催されたACP Internal Medicine Meeting 2025と、CalgaryでのPOCUS teamへのobservershipという、計2週間の海外研修に参加させて頂きました。

＜Precourse: 4/1-2＞
Precourseは「Diabetes for Internal Medicine Physicians」と「Geriatrics and Palliative Medicine for Internal Medicine Physicians」の2講座を受講しました。朝8時から夕方5時まで続く怒涛のレクチャーを英語で受け続けるのはタフで心が折れそうでしたが、事前配布された200ページに及ぶpdf資料が非常に充実しており、それを頼りに何とかついていくことができました。いずれも各領域の最新のエビデンスを網羅したReview形式で、若手よりもベテランの先生方が多く参加されていたのが印象的でした。
Diabetesのセッションでは、アメリカにおける課題といえる肥満症に関連したGIP/GLP-1受容体作動薬の話題が多く取り上げられ、また持続血糖測定(CGM)がOTCで広く活用されていることに驚きました。
Geriatric Medicineのセッションでは、「5Ms」に基づいたアプローチを学びました。フレイルに対する薬物治療の可能性が提示される一方で、認知症周辺症状に対して日本で頻用されているような漢方薬は当然紹介されず、高齢者診療において日本からエビデンスを発信する余地を感じました。
講師の誕生日にサプライズでケーキが用意されているような遊び心がありつつも、参加者が活発に質問し熱心に学んでいる様子に刺激を受けました。

＜Internal Medicine Meeting 2025: 4/3-5＞
Precourseより大幅に参加者が増え、世界最大の内科学会の名に恥じない規模に圧倒されました。日本では考えられない大きさの大会場で行われたオープニングセレモニーで、Tuckson先生がトランプ政権下でのワクチン問題や誤情報の拡散について「Physicianはmedical misinformationの最前線にいる」と力強く訴え、会場全体が割れんばかりのスタンディングオベーションで応えた場面に強く感銘を受けました。その後も数々のセッションに参加し、緩和ケア・がん検診・臨床推論・診断エラーなどの日常診療に直結するテーマはもちろん、関心のある医学教育、さらにはアメリカならではのカンナビノイドに関する話題など、非常に幅広いトピックに触れることができました。また広大な展示ブースも印象的で、アメリカ海軍/陸軍のコーナーがあったり、日本発の薬剤が大々的に宣伝されている場面もあったり等、興味深く拝見しました。
また、懇親会では普段なかなかお会いできないような先生方と交流させていただき、診療へのモチベーションが大いに高まりました。Centor scoreで著明なCentor先生、現職場上司の指導医であったMeyer先生にお会いできたことがハイライトです。
参加者の多くがGeneralistであったからか、学会全体に流れる空気感が心地よかったです。そのような環境は日本の現状とは隔世の感を受けましたが、日本のGeneralismを発展させるべく頑張ってゆく決意を新たにしました。

＜University of Calgary POCUS team: 4/7-11＞
Foothills Medical CentreでIrene Ma先生率いるPOCUS teamの見学をさせて頂きました。Ma先生＋レジデント4名の計5名から成るPOCUS専門のコンサルテーションチームで、相談のあった患者さんに関してルーチンで肺エコー/心エコー/IVC・JVP測定によるvolume評価を行ったり、エコーガイド下で腹腔/胸腔穿刺を行ったり、時には皮膚軟部組織を見たりと、全身のエコーを数多く経験させて頂きました。医師の働き方や医療環境の違いに驚かされることもありましたが、Ma先生が「Beautiful!」「Perfect!」など多彩な表現でレジデントを褒められ、隙間時間にすかさずシミュレーションやレクチャーをされる指導スタイルは、医学教育に関心がある私にとって多いに刺激になりました。また、自身が日常的にエコーを用いる機会が少ないことを反省し、POCUSが総合内科・家庭医療という自分の診療領域と非常に親和性が高いことを改めて実感しました。帰国後にPOCUSを日々の診療や教育にぜひ取り入れていこうと決意しました。
Ma先生やレジデントの皆さんの素晴らしいホスピタリティにも感激し、充実した楽しい1週間を過ごすことができました。

総じて、大変刺激的な2週間を過ごさせて頂きました。英語での議論についていくための自身の語学力不足を反省する一方で、臨床面に関しては日本での実践も必ずしも海外基準に劣っていないことも多いのではないかと感じ、英語学習や日々の臨床を引き続き頑張る励みになりました。
改めまして、このような貴重な機会をくださった矢野晴美先生をはじめとするJapan Chapterの先生方、年度初めという忙しい時期にもかかわらず2週間の不在を許してくださった職場の皆さま、そして2週間一緒に濃い時間を過ごさせて頂いた相棒の石原洋先生に、深く感謝申し上げます。本当にありがとうございました。

 

 

ACP Annual Conference Internal Medicine Meeting 2025

Rising Star Scholarship Program Awardee

I was truly honored to be selected as the awardees for the Rising Star Scholarship Program 2025. This provided me with a valuable opportunity to participate in two weeks of international training: attending the ACP Internal Medicine Meeting 2025 in New Orleans and joining an observership with the POCUS team in Calgary. This experience greatly influenced my outlook as a generalist.

＜Precourse: April 1-2＞
Prior to the main meeting, I attended two precourse sessions: “Diabetes for Internal Medicine Physicians” and “Geriatrics and Palliative Medicine for Internal Medicine Physicians.” Attending full-day lectures in English from 8 a.m. to 5 p.m. was highly demanding, but the well-prepared 200-page PDF materials provided in advance enabled me to keep up with the sessions. Each course offered a comprehensive review of up-to-date evidence in the field, and many experienced physicians were in attendance. In the Diabetes session focused on topics such as GIP/GLP-1 receptor agonists related to obesity, and the widespread over-the-counter use of continuous glucose monitoring (CGM) devices in the U.S., which was surprising to me. In the Geriatrics session, I learned an approach based on the “5Ms.” While new pharmacologic approaches to frailty were introduced, there was no mention of Kampo medicine, which is often used in Japan to treat behavioral and psychological symptoms of dementia (BPSD). This highlighted for me the potential to contribute Japan-generated evidence to the global geriatric field. Despite the intense schedule, the friendly atmosphere, including a birthday celebration for one of the instructors and active audience participation, made the sessions engaging and inspiring.

＜Internal Medicine Meeting 2025: April 3-5＞
Compared to the precourse, the number of participants increased significantly, and the scale of the event was impressive. At the opening ceremony, Dr. Tuckson addressed the challenges of vaccine misinformation under the Trump administration, emphasizing that physicians are on the front lines of medical misinformation. His speech received a standing ovation, which was deeply moving. Over the next few days, I attended a wide range of sessions, covering topics directly applicable to everyday clinical practice—such as palliative care, cancer screening, clinical reasoning, and diagnostic error—as well as subjects aligned with my personal interests, including medical education and uniquely American themes such as cannabinoid use in clinical care. The large exhibition hall featured booths from the U.S. military and promotional displays for Japanese-developed drugs, which were interesting to observe. At the networking events, I had the rare opportunity to connect with distinguished physicians, which greatly boosted my motivation. A particular highlight was meeting Dr. Centor—well-known for the Centor score—as well as Dr. Meyer, a former mentor of my current supervisor.
The overall atmosphere was warm and collegial, likely due to the many generalists in attendance. It was a refreshing contrast to the environment in Japan and strengthened my commitment to advancing generalism back home.

＜University of Calgary POCUS team: April 7-11＞
Following the ACP meeting, I observed the POCUS consultation team at Foothills Medical Centre in Calgary, led by Dr. Irene Ma. The team consisted of Dr. Ma and four residents and conducted routine assessments, including lung and cardiac ultrasound, IVC/JVP evaluation, and ultrasound-guided procedures. I was able to gain hands-on experience with a wide range of scans. I was surprised by the differences in medical practice and working culture, but also deeply impressed by Dr. Ma’s positive teaching style—using varied praise such as “Beautiful!” and “Perfect!” to encourage residents, and seamlessly integrating simulations and mini-lectures into the day’s schedule. At the same time, This experience also made me reflect on my own limited use of ultrasound and helped me realize the strong relevance of POCUS in general internal and family medicine. I am now determined to incorporate POCUS into my daily practice and teaching in Japan. I was also deeply touched by the warm hospitality of Dr. Ma and the entire POCUS team, which made for a thoroughly enriching and enjoyable week.

Overall, these two weeks were an eye-opening and highly stimulating experience. While I recognized the need to improve my English for international discussions, I also felt confident that many aspects of clinical practice in Japan are on par with global standards. This realization encouraged me to continue improving both my language skills and clinical practice.

Finally, I sincerely thank Dr. Harumi Yano and the ACP Japan Chapter for this opportunity, my colleagues at Fukuchiyama City Hospital for supporting my absence during a busy time of year, and Dr. Yo Ishihara, who shared this enriching experience with me. Thank you very much.

私は今回Rising Star Scholarship Programの恩恵を受け、第二期生として、米国ニューオーリンズで開催された
米国内科学会（ACP）総会、カナダ国アルバータ州カルガリー大学でのポイントオブケア超音波の研修、合計2週間の海外渡航を経験させていただきました。短期留学と言っても良いのではないかと思うくらい、毎日が濃密で、新しい出会いと学びに満ちていました。ACPは想像をはるかに上回る大規模な組織で、今回の総会は世界中の多数の医療従事者が一つの会場に集結して情報交換と学習の時間を共にすることができる壮大な学会でした。多くの他国の先生方とお話することができ、連絡先を交換し、さらに研究の相談にも乗っていただきました。学会スタッフも親切な方ばかりで、講義資料を求める相談に、丁寧にご対応くださりました。感謝申し上げます。

学会ではプレコースから参加し、主に栄養学とプライマリの薬物治療を学びましたが、演者の先生方が原著論文、特にRCTの条件設定をサラリと会話の中に織り交ぜてお話しされていたことが印象的でした。朝8時から17時までの講義でしたが、終始うきうきと拝聴し、夕方になればなるほど面白く、疲労はほとんど感じませんでした。何がわかっていて、何がわかっていないのか、そして何がわかっていないから、どこまで言及していいのか、を相当意識されているようにも思われ、その点が勉強になりました。私の拙い質問にも快くお答えくださり感激しました。ほか身体診察と超音波検査の実技講習（Clinical Skills）にも参加しましたが、やはり科学的根拠が重視された内容で、直接丁寧なご指導をいただきました。チームディスカッションのセッションにも参加しましたが、米国と日本の抱える問題は共通しており、例えば、身体診察が疎かでCTなどの検査が優先されベッドサイドに行く時間が減っている、などの問題点を共有しました。講演や参加者のお話を伺う限り、日本も米国も、医療の水準や、抱えている臨床的・教育的問題は大きくは変わらないと思われました。逆にいえば日本の医療が高水準であることは、まさに先輩たちの功労によって成立しているのだろうと推察しました。

カルガリー大学での超音波診療の研修は、Irene Ma先生とPOCUSチームの皆様の時間を頂き、超音波の基本的事項、実際の診療を見学させて頂きました。Ma先生、チームの先生方が慈悲深く接してくださりました。特にvolume評価について所見の論理的な解釈方法、超音波プローブの当て方まで直接教わることができ、またとない勉強の機会でした。私は超音波の基礎を学び研究のご相談をしたいと考えておりましたので、渡航の目的を完全に果たすことができました。

言語の壁は私の個人的問題でもあり、毎日学習を継続していたにもかかわらず、十分ではなかったことを悔みましたが、万人に通じることで重要なことは英語や知識よりも、己の情熱や、礼節・寛容・利他の精神であると思われました。ACP総会開会式でのReed Tuckson先生の演説は、他人のために尽くす情熱を思い出し、患者さんはもちろん、同僚も含めて周りの人たちみんなを大事にしようという、医療者の初心を振り返るよう激励する内容も含まれていましたが、こうしたマインド、すなわち広義の「思いやり」こそが、重要であり忘れてはならないのだと振り返りました。移動中のタクシーや、レストラン、空港の待合室でも、海外の人たちとお話しする中で同様のことを感じ、自らの生活を反省する良い機会になりました。

私がこの2週間を通し学んだことは膨大でとても書ききることはできませんが、特に考えたことは、日本の医療が先人達の努力のお陰で高い水準を保っているものの、これが今後維持・発展されるか否かは私たちの世代にかかっている、ということです。

カナダで最大の発展を遂げているカルガリーには、建設途中のビルが多数ありました。寒い環境の中で、美しい自然を尊重しながら、かなりの速度で社会的・文化的・経済的に発展しているように見えました。かつ、他文化を受け入れて、尊重し、うまく調和しているように思われました。日本は人口減少も影響してか、将来に対して悲観的なムードが漂っているということはイプソス幸福感調査2025でも示されているようですが、ただこれまでの日本の経済発展、医療の進歩は先人の知的で勇敢な行動が積み重なって成熟したもので、私はその上に立たせてもらっているのだと理解しました。次の10年20年をどうするかというと、先人達と同じかそれ以上に努力し、世界に進出し発信して、よりよいものを取り入れて、日本の患者さんたちが高水準の治療を受け続けられるように、維持・発展続けるということに尽きるのだろうと思い、ある種の危機感を抱きました。語学学習はもちろんですが、多くの臨床経験を重ねて科学的に検討し、外界と発信・交通して仲間と共有することが私の課題と思われ、そしてこの課題が自分の中で明確になったことは、今回の渡航で得られた最大の恩恵であると考えています。

今回の海外渡航・勉強、交流の機会をくださったACP日本支部長の矢野晴美先生、ACP日本支部の先生方・スタッフの皆様、カナダ国カルガリー大学Irene Ma先生、POCUSチームの皆様、今回の勉強のための渡航を快諾くださった国際医療福祉大学成田病院の消化器内科学教室の先生方、緩和ケアチームの皆様、第1期として前例を築いて下さり、背中を押してくださった田中幸介先生、名嘉祐貴先生、お会いくださった全ての方々に心から感謝を申し上げます。一緒に渡航いただいた稲葉哲士先生は、総合診療業界で若手トップを走っておられるまさに“Rising Star”であり、長期間寝食を共にし、貴重な思い出を共有できたことは非常に光栄に思います。渡航の意欲を掻き立ててくださったのは黒川清先生の著書「考えよ、問いかけよ」でした。2回読みました。Rising Star Programという名前の奨学金制度ですが、私にとってはRising “SUN” programで、目が覚めたような気分です。お世話になった皆様ありがとうございました。どうか、引き続き、今後ともよろしくお願いいたします。

Rising Star Scholarship Program Awardee

Thanks to the Rising Star Scholarship Program, I had the invaluable opportunity to attend the American College of Physicians (ACP) Annual Meeting in New Orleans and to participate in a two-week overseas training, including a point-of-care ultrasound (POCUS) course at the University of Calgary, Alberta, Canada. Each day was filled with rich learning and meaningful encounters—it truly felt like a short-term study abroad program.

The ACP was far larger than I had imagined, gathering healthcare professionals from around the world in one venue for a shared learning experience. I was able to connect with many international physicians, exchange contacts, and even discuss research ideas. The staff were also kind and helpful, supporting my requests for lecture materials. I joined the pre-courses and focused on clinical nutrition and pharmacotherapy in primary care. I was impressed by how the speakers effortlessly incorporated original research and RCT design concepts into their talks. The full-day lectures never felt tiring; rather, they grew more engaging as the day progressed. What stood out was the speakers’ clarity on what is known, what remains unknown, and where to draw the line—something I found very educational. They kindly answered my questions, despite my limited English. I also took part in hands-on sessions in physical examination and ultrasound, both of which emphasized evidence-based practice. During a group discussion, participants shared concerns common to both the U.S. and Japan, such as the declining use of bedside skills due to an over-reliance on imaging. These sessions reminded me that the high standards of Japanese medicine today are built on the tireless efforts of our predecessors. At the University of Calgary, Dr. Irene Ma and her POCUS team generously shared their time to teach me the fundamentals of ultrasound and allowed me to observe real clinical practice. I was especially grateful for the detailed instruction on volume assessment and probe handling—this was a unique and inspiring learning opportunity that fully met my training goals.

Although I continued studying English every day, I regret that I was not sufficiently fluent. However, I realized that what matters more than language is one’s passion, humility, empathy, and altruism. Dr. Reed Tuckson’s opening speech at the ACP meeting was a powerful reminder to serve others wholeheartedly and to care deeply for both patients and colleagues. Even in casual conversations during taxi rides or at restaurants and airports, I had the chance to reflect on my own values and attitudes.

One major takeaway from these two weeks was the realization that while Japan maintains high medical standards thanks to our forerunners, the responsibility for sustaining and advancing this legacy now lies with our generation. Calgary is undergoing rapid development despite its cold climate, with many buildings under construction and a society that balances economic growth with environmental respect and multicultural harmony. In contrast, Japan is facing population decline and a sense of pessimism, as noted in the 2025 Ipsos Global Happiness Survey. Nonetheless, our country’s past achievements in medicine and economy are the fruits of courageous, intellectual action, and I am standing on those foundations. Looking ahead, I believe we must work as hard as or even harder than our predecessors, engage globally, and incorporate new ideas to ensure that Japanese patients continue to receive top-level care. In addition to language proficiency, we need to deepen our clinical experience, evaluate it scientifically, and share our findings with others. Clarifying this personal mission was the most meaningful outcome of this journey.

I would like to express my sincere gratitude to Dr. Harumi Yano, Governor of ACP Japan Chapter; all ACP Japan faculty and staff; Dr. Irene Ma and the POCUS team at the University of Calgary, Foothills Medical Centre; my colleagues at IUHW Narita Hospital; Drs. Kosuke Tanaka and Yuki Naka for paving the way as first-year scholars; and everyone I met along the way. Special thanks to Dr. Satoshi Inaba, a true “Rising Star” in general medicine, with whom I had the honor of sharing this unforgettable journey. Lastly, I was inspired to apply by Dr. Kiyoshi Kurokawa’s book, “Kangaeyo, Toikakeyo (“Think, Ask”)”, which I read twice. Though officially named the Rising Star Program, to me, it was more like the “Rising Sun”—a moment of awakening. I am deeply thankful to all who made this possible and humbly ask for your continued guidance in the future.

ACP日本支部年次総会・講演会2023  医学生特別賞を受賞して国際医療福祉大学 医学部医学科4年生の大橋龍人様から、Internal Medicine Meeting 2024ポスター発表の体験レポートを頂きましたのでご報告いたします。

2024年4月18日～4月20日にアメリカ・ボストンで開催された、米国内科学会年次総会2024 のNational Poster Competitions for Medical Students and Resident/Fellow Membersに参加させていただきましたので、その様子をご紹介させていただきます。

まず参加のきっかけですが、米国内科学会日本支部総会2023においての事例報告で医学生特別賞を頂いたことです。毎年、各支部の一般演題発表優勝者はChapter Winnersとして米国本部のポスター発表に参加できますが、この権利は学生会員（Medical Student Member）もしくは準会員（Residents/Fellows-in-Training Member）に限定され、昨年度日本支部の黒川賞を受賞された2名の先生方は該当の会員種類の対象外だったため、私が発表の機会を頂いた次第でございます。

特典として総会へ無料で招待していただいたため、私の発表自体は３日目の午前中でしたが全日参加させていただきました。幕張メッセの約８倍の大きさのある会場、Boston Convention and Exhibition Centerの全体を使った総会は私の知る中で最も先進的で雄大な学会でした。また、日本を含め米国以外からも様々な国の支部から先生方がいらしており、特に開会式ではオリンピック選手団の一人になったような気持ちで日本支部の先生方と行進をしておりました。参加者全員が集い、普段の努力や貢献を讃えあう時間は大変素敵で、忘れることのできない光景でした。

米国内科学会日本支部総会2023はオンラインで行われましたが、世界中の先生方のご尽力のお陰でコロナ禍が収束に向かい、米国内科学会年次総会2024は現地開催であり私のポスター発表も対面形式でした。オフライン発表の魅力は周りの発表者や来場者とのコミュニケーションが取りやすいことで、世界中の医師や医学生と交流しながらコロナ禍が去った喜びを噛み締めておりました。National Poster Competitionsに参加していた医学生は、各州や国の支部から選ばれた素晴らしい研究をしている方々です。研究の質やプレゼンは医学生のものとは思えないほどに洗練されており、どれも聴いていて勉強になるものばかりでした。発表当時は臨床実習が丁度始まる時期であり、そのタイミングで質の良い研究に触れられたことは大変なモチベーションとなりました。

今回、“Successful preventive strategies for COVID-19 at the interactive festival in Shibuya, Tokyo”という演題のもと、2022年9月3〜4日に渋谷で開催された音楽祭において実施した感染対策について事例報告をしました。当時はCOVID-19第7波の時期で東京では緊急事態宣言が発令され、音楽祭を含むほとんどの文化的イベントは中止を余儀なくされていました。しかし、「正しい感染対策をしていればインタラクティブなイベントでも開催できる」という仮定のもとエビデンスに基づく入念な対策を施した結果、2万人が集まるイベントにてクラスターを起こさずに開催できたというものです。この事例報告を海外の先生方に評価いただきディスカッションをした時間は大変有意義なものであり、発表者である私もこの報告の価値を新視点から再認識することができました。この時間のためだけでも渡米する価値があり、非常に良い経験となりました。

最後に、円安が進む昨今、３日間とはいえ米国への渡航費・宿泊費は学生にとって大きな出費でしたが、米国内科学会日本支部からの渡航支援は大変有り難いものでした。また、発表の準備や渡米に関してお世話になりました全ての皆様に改めて深謝申し上げます。

I had the privilege of participating in the National Poster Competitions for Medical Students and Resident/Fellow Members at the American College of Physicians (ACP) Annual Meeting 2024, held from April 18 to April 20 in Boston, the US. I would like to provide an account of this experience.

The opportunity to participate arose from receiving the Special Award for Medical Students at the ACP Japan Chapter Annual Meeting 2023. Each year, winners of general presentations from various chapters are eligible to participate in the poster presentations at the US headquarters as Chapter Winners. This opportunity is restricted to student members (Medical Student Members) or resident/fellow-in-training members, and since the two recipients of the Kurokawa Award from the Japanese chapter last year did not fall under these membership categories, I was given the chance to present.

As a benefit, I was invited to the meeting free of charge. Although my presentation took place on the morning of the third day, I attended the entire conference. The meeting, held at the Boston Convention and Exhibition Center, which is approximately eight times the size of Makuhari Messe, was one of the most advanced and grand conferences as far as I know. Additionally, attendees from various countries, including those outside the US, were present. Particularly during the opening ceremony, I felt like a member of an Olympic delegation as I marched with doctors from Japanese chapter. The time spent celebrating each other’s efforts and contributions was truly wonderful and left an unforgettable impression.

While the ACP Japan Chapter Annual Meeting 2023 was held online, the ACP Annual Meeting 2024 was an in-person event, and my poster presentation was also in-person. One of the advantages of offline presentations is the ease of communication with other presenters and attendees. I cherished the opportunity to interact with physicians and medical students from around the world and to celebrate the end of the COVID-19 pandemic. The medical students participating in the National Poster Competitions were conducting outstanding research selected from various states and countries. The quality of their researches and presentations were so refined that it was hard to believe they were students, and each presentation was educational. At the time of my presentation, I was at the beginning of the clinical clerkship, and encountering such high-quality research led to be a tremendous source of motivation.

I presented a case study titled “Successful Preventive Strategies for COVID-19 at the Interactive Festival in Shibuya, Tokyo.” This presentation detailed the infection control measures implemented at the music festival held in Shibuya on September 3-4, 2022. At that time, during the 7th wave of COVID-19, Tokyo was under the state of emergency, and most cultural events, including the music festival, were canceled. However, based on the assumption that interactive events could be held with proper infection control measures, meticulous evidence-based measures were implemented. As a result, the event, which drew 20,000 attendees, was conducted without causing a cluster outbreak. The evaluation and discussion of this case study with international colleagues were highly meaningful, and I was able to reassess the value of the case report from a new perspectives. The experience of traveling to the US for this alone was worthwhile, and it proved to be an extremely valuable experience.

Lastly, given the recent depreciation of the yen, the travel and accommodation expenses for a three-day trip to the U.S. were significant for a student. I am very grateful for the travel support provided by the ACP Japan Chapter. I also wish to extend my sincere gratitude to everyone who assisted with the preparation for the presentation and the travel arrangements.

福島県立医科大学総合内科の庄子啓先生から、Internal Medicine Meeting 2024 Doctor’s Dilemmaの体験レポートを頂きましたのでご報告いたします。

この度、私は米国内科学会日本支部関係者の皆様からの旅費助成を含めた多大なるご支援のもと、アメリカボストンのACP学会で開催されたクイズ大会、Dr’s. Dilemmaに参加しました。医療に関する幅広い知識が問われるこの大会は、私にとって非常に刺激的な経験となりました。米国の学生たちが、様々な問題に対して自信を持って素早く回答していく姿が印象的で、その対応力に感銘を受けました。特に、日本の医学教育では、体系的に知識を習得し、じっくり考えた上での診断力を養うことが重視されていますが、一方で、米国の教育では実践的な応用力と即応性が重視されていることを感じました。また基礎医学及び公衆衛生に関連する問題も多数出題され、これらの領域の米国における重要性を知り、また日々の臨床と結びついていることを垣間見られるよい機会になりました。

このような異文化での学びは、私にとって新鮮であり、日本の医療現場においても適用できる新しい視点を得る機会となりました。また、米国の参加者たちとの交流を通じて、国際的な医療事情や異なる診療アプローチについて知りました。自己の成長につながる有意義な時間を過ごせました。この経験を踏まえ、日本においてもより多角的なアプローチを学び実践することで、患者への医療提供に貢献したいと考えています。

With the generous support of the American College of Physicians (ACP) Japan Chapter officials, including travel grants, I was able to attend the quiz competition, Dr’s. Dilemma, held at the ACP Society in Boston, USA. This competition, which tested a wide range of medical knowledge, was a very exciting experience for me. I was impressed by the way American students confidently and quickly answered various questions and was impressed by their responsiveness. In particular, I was impressed by the fact that Japanese medical education emphasizes the acquisition of knowledge in a systematic manner and the development of diagnostic skills based on careful consideration, whereas U.S. education emphasizes practical application and immediate response. In addition, there were many questions related to basic medicine and public health, and it was a good opportunity for me to learn about the importance of these areas in the U.S. and to get a glimpse of how they are connected to daily clinical practice.

Learning in such a different culture was refreshing for me and provided me with an opportunity to gain new perspectives that can be applied in the Japanese medical field as well. I also learned about international medical situations and different approaches to medical care through exchanges with American participants. It was a meaningful time that led to my personal growth. Based on this experience, I would like to contribute to the provision of medical care to patients in Japan by learning and practicing a more multifaceted approach.

市立福知山市民病院の奥村尚稔先生から、Internal Medicine Meeting 2024 Doctor’s Dilemmaの体験レポートを頂きましたのでご報告いたします。

この度、光栄にもInternal Medicine Meeting 2024のDoctor’s Dilemmaに参加させていただきました。初めての国際学会でもあり、Doctor’s Dilemmaだけでなく、規模の大きさ、学会の雰囲気、そして講演などの全てが刺激的に感じました。Doctor’s Dilemmaに関しては、残念ながら望んだ成績を残すことができませんでしたが、学会参加を通して多くのことを学びました。

現地の教育講演は、非常に高い質のもので、基礎的な内容から最新の臨床研究までを解説するものが多く、大変勉強になりました。また、展示会場では最新の医療機器はさる事ながらら、JAMA Internal MedicineのEditor in Chiefなどの医療界のTop Runnerとお話しをする機会を得られ、国内学会ではなかなか得られない経験をすることができました。

医学知識や英語力などの向上に努め、今度は研究発表などで国際学会に参加できるように精進していきます。

米国内科学会日本支部の関係者の皆様には、このような貴重な機会を頂き、心より感謝申し上げます。また、旅費助成もして頂きありがとうございました。

I had the honor of participating in the Doctor’s Dilemma at the Internal Medicine Meeting 2024. This was my first international conference, and I found everything—from the scale and atmosphere of the conference to the lectures—extremely stimulating. Although I was unable to achieve the desired results in the Doctor’s Dilemma, I learned a great deal through my participation in the conference.

The educational lectures on-site were of very high quality, covering topics from basic content to the latest clinical research, and I found them incredibly informative. Additionally, at the exhibition hall, I had the opportunity to interact with top leaders in the medical field, such as the Editor-in-Chief of JAMA Internal Medicine, and to see the latest medical equipment. These experiences were invaluable and something I rarely encounter at domestic conferences.

I will continue to strive to improve my medical knowledge and English proficiency so that I can participate in international conferences with my own research presentations in the future.

福島県立医科大学附属病院の内藤 翔太郎先生から、Internal Medicine Meeting 2024Doctor’s Dilemmaの体験レポートを頂きましたのでご報告いたします。

先日ボストンで開催されたInternal Medicine Meeting 2024のDoctor’s Dilemmaに、日本チームの一員として参加しました。私にとって初めての国際学会であり、様々なことを感じ、学ぶことができましたが、特に米国レジデントの知識の幅と質に最も感銘を受けました。

Doctor’s Dilemma日本予選は主にMKSAPに準拠した臨床的な問題で構成されています。一方で、本戦ではUSMLE STEP1-2に類似した問題が出題され、より基礎的な内容の問題が多く含まれています。臨床の知識に留まらず、基礎医学や統計学、社会保険制度など、日本のレジデントが疎かにしがちな分野からも問題が出され、それを当たり前のように正答していく米国レジデントのチームには終始圧倒されました。

希少疾患や難治症例の治療では、病態生理に基づいたアセスメントや効率的な文献検索が必要になることがあります。臨床能力を向上させるため、基礎医学や統計学の知識を実用的な知識として日常臨床に応用できるように、知識の復習と補強を行おうと強く思いました。

今回の経験を一言で表せば、「井の中の蛙大海を知らず」に尽きると思います。海外のレジデントの知識や視野の広さを知り、自分の改善点をメタ認知することができた良い機会になりました。今後も機会があれば、海外学会には積極的に参加したいと思います。

I recently attended the Doctor’s Dilemma at the Internal Medicine Meeting 2024 in Boston as a member of the Japanese team. This was my first international conference, and it provided me with a wealth of learning experiences and insights. Among these, what impressed me most was the breadth and quality of medical knowledge possessed by the U.S. residents.

The Doctor’s Dilemma Japan chapter primarily consists of clinical questions in accordance with MKSAP. In contrast, the main competition features questions similar to those of USMLE Step 1 and Step 2 but with a greater emphasis on basic content. These questions extended beyond clinical knowledge to cover areas often overlooked by Japanese residents, such as basic medicine, statistics, and social insurance systems. I was overwhelmed by the U.S. residents who answered these questions correctly and effortlessly, as if it were second nature to them.

Treating rare diseases and difficult cases sometimes requires assessments based on pathophysiology and efficient literature searches. This realization highlighted for me the importance of strengthening my foundational knowledge in basic medicine and statistics. By doing so, I can enhance my clinical skills and apply this practical knowledge more effectively in my daily practice.

Reflecting on my experience, if I had to sum it up in one phrase, it would be “a big fish in a small pond.” This conference was an excellent opportunity for me to understand the breadth of knowledge and perspective of overseas residents and to recognize my own areas for improvement.

This experience has inspired me to continue seeking opportunities for international engagement. I now recognize the significant benefits of participating in overseas academic conferences, which not only broaden my knowledge but also allow me to gain new perspectives and insights. Moving forward, I am determined to actively pursue and participate in such conferences whenever possible, as they are instrumental in my growth as a medical professional.
