2026年4月に米国で開催されたAmerican College of Physicians Internal Medicine Meeting 2026（IMM 2026）に参加し、ポスター発表を行いました。

今回の参加は、ACP日本支部 年次総会・講演会2025において黒川賞を受賞したことがきっかけです。日本支部から渡航助成を賜り、心より感謝申し上げます。本学会での発表にあたっては、抄録の新規提出と採択が必要であったため、既存の研究内容を再解析し、モデルからスコアリングへと再構成して応募しました。その結果、Early Career Physician Competitionのポスターセッションに採択され、渡米の機会を得ることができました。

会場は非常に大規模で活気にあふれ、世界各国から多くの医師が参加していました。ポスター発表の待機時間には、同様に発表を控えた参加者と交流する機会もあり、国際学会ならではの雰囲気を実感しました。発表は4分間のプレゼンテーションと質疑応答で構成され、研究の今後の展開や方法論に関する指摘をいただきました。日頃から国内でも同様の指摘を受ける機会はありますが、英語でそれらに即座に対応する難しさを実感し、準備していた内容を超えた議論に戸惑う場面もありました。本経験を通じて、研究手法についてより深く学ぶ必要性とともに、日頃から英語で説明・議論する訓練の重要性を強く認識しました。

また、現地での生活やコミュニケーションにおいて言語の壁を感じる場面もありましたが、それ以上に「伝えようとする姿勢」の重要性を実感しました。日本支部の先生方との交流も大変有意義であり、今後の活動への大きな励みとなりました。

本経験を今後の臨床・研究活動に活かし、引き続き国際的な発信に取り組んでいきたいと考えています。このような貴重な機会をいただいたすべての皆様に、深く感謝申し上げます。

最後に、英語のハードルを理由に海外学会への参加をためらっている先生方に向けて、私自身の経験を共有したいと思います。私は日本で生まれ育ち、長期の海外経験はありません。高校時代に約10日間の米国ホームステイを経験した程度で、その後も短期間の海外旅行にとどまります。受験期には英語を学習していたものの、大学入学後は他の学問を優先し、英語から離れていました。英語学習を再開したのは30代に入ってからで、基礎から改めて取り組みました。

学習にあたっては、発音や文法といった基礎を整理した上で、オンライン英会話を用いた実践的なトレーニングを継続しています。学習再開から約4年が経過し、現在も週3回程度の頻度で継続しています。そのような背景の私でも、今回のポスター発表において、ジャッジの先生に英語が適切に伝わっていたかを確認したところ、「問題なく理解できた」との評価をいただくことができました。

もちろん不安もありましたが、同じ施設から参加した同僚や後輩、さらに現地で支えてくださった日本支部の先生方の存在に大きく支えられました。また、近年はAI翻訳などの技術の進歩により、言語の壁を補う手段も広がっています。

海外学会への参加は決してハードルの低いものではありませんが、日本で主に研鑽を積んできた方であっても、準備と工夫、そして周囲の支えがあれば十分に挑戦可能であると実感しました。本経験が、同様の不安を感じている先生方の一助となれば幸いです。

I attended the American College of Physicians Internal Medicine Meeting 2026 (IMM 2026) in the United States and presented my research in a poster session.

My participation was made possible by receiving the Kurokawa Award at the ACP Japan Chapter Annual Meeting 2025. I am sincerely grateful for the travel grant provided by the Japan Chapter. To present at the meeting, it was necessary to submit a new abstract and obtain acceptance. I reanalyzed my previous research and reconstructed it into a scoring system, which was accepted for presentation in the Early Career Physician Competition. This opportunity allowed me to attend the meeting and share my work internationally.

The conference was large and active, with participants from around the world. During the poster session, I had opportunities to interact with other presenters while waiting for my turn, which gave me a strong sense of the international academic environment. The presentation consisted of a four-minute talk followed by discussion with judges. I received valuable feedback on the future direction of my research and its methodology. Although I am accustomed to receiving similar comments in domestic settings, I found it challenging to respond promptly in English, particularly when discussions extended beyond what I had prepared. This experience highlighted the need to further strengthen my understanding of research methodology, as well as the importance of regularly practicing how to explain and discuss my work in English.

While I did encounter some language barriers in daily communication, I also realized that a willingness to communicate clearly is just as important as language proficiency. Interactions with members of the ACP Japan Chapter were also highly meaningful and motivating.

Finally, I would like to share a message with those who may hesitate to participate in international conferences due to language concerns. I was born and raised in Japan and have limited experience abroad. My only extended exposure to an English-speaking environment was a short homestay in the United States during high school, along with a few brief personal trips overseas. Like many physicians, I studied English mainly for entrance examinations, but I did not actively continue learning after entering medical school. I resumed studying English in my 30s, starting again from the basics.

My approach was to first strengthen fundamental skills such as pronunciation and grammar and then develop practical communication skills through online conversation practice. I have continued this training for about four years, currently about three times a week. Despite this background, when I asked one of the judges whether my English had been clear, I was told that it was well understood without any issues.

Although I did feel some anxiety, I was greatly supported by colleagues and junior members from my institution, as well as members of the ACP Japan Chapter who kindly assisted me on site. In addition, recent advances in AI-based translation tools have made it easier to overcome language barriers in practical settings.

Participating in an international conference may seem challenging, but I found it to be an achievable goal with adequate preparation, persistence, and support. I hope that my experience will encourage others who feel hesitant to take a step forward.

この度、2026年度Rising Star Scholarship ProgramのAwardeeとして、米国San Franciscoで開催されたACP Internal Medicine Meeting 2026および、カナダCalgaryでのPoint-of-Care Ultrasound (POCUS) 研修に参加させていただきました。

＜Pre-course: 4/14-15＞
Pre-courseでは、「Nutrition in Practice: Evidence-Based Strategies for the Internal Medicine Physician」と「Cardiology for the Internal Medicine Physician 2026: “The Key Points”」のコースを受講し、8時から17時まで講義を受けました。NutritionではCulinary Medicine、サプリメント、低栄養、肥満・糖尿病に対する食事療法、経腸・静脈栄養、微量栄養素欠乏などが含まれており、特に情報にあふれた現代において、医療者が普段からエビデンスに基づいた栄養に関する正確な知識を患者に伝えることの重要性を感じました。Cardiologyでは急性冠症候群、心房細動、心不全、術前心血管評価、心臓画像、弁膜症、高血圧、心電図異常など、内科医が日常診療で頻繁に遭遇するテーマを整理し直すことができました。どちらのコースでも、ガイドラインの推奨度やRCTなどの根拠を示しながら講義が進められていたことが印象的でした。

＜ACP総会 2026: 4/16-18＞
本会では、AIに関するセッションが数多く設けられていました。OpenEvidenceはUSMLEで100%の正答率であり知識面でAIに勝つのは難しい一方で、それでも多くの人々は人間の医師に診てもらいたいと考えているという議論もあり、医師の価値を改めて考えさせられました。業務効率化のためのAIの使用によりバーンアウトが防げること、患者のためのAIツールの発達など、様々な視点から今後AIが医療においてどのような役割を果たすかが考察されていました。

また、身体診察のブースでハンズオンの講義を受けた際には、Frameworks for Internal Medicine の著者であるDr. André Mansoorに直接JVPの診察について教わりました。聖路加国際病院の指導医が彼と同じ木製のベル型の聴診器の使用を推奨しており、自分も持っていることをお伝えしたところ、大変驚いておられました。講義内容も素晴らしく、Physical Diagnosis PDXやStanford Medicine 25といったサイトには同様の身体診察の動画が豊富に掲載されているため、今後も参考にしつつ研鑽を続けようと思いました。

その他にもACP本会では多くの出会いがあり、セッション中に隣に座った先生が何気なく話しかけてくださり、そのまま名刺交換につながることもありました。学会は講演を聴くだけの場所ではなく、同じ関心を持つ医師と出会い、将来のキャリアにつながる関係を築く場でもあるのだと実感しました。Rising Star Scholarship Programを通じてこのような環境に身を置けたことは、今後国際的な場で学び続けるうえで大きな励みとなりました。

＜University of Calgary POCUS研修：4/20-24＞
ACP後には、CalgaryのFoothills Medical Centreにて、Dr. Irene Ma先生率いるPOCUS teamで研修をさせていただきました。私はこれまで日常診療の中でエコーを使用する機会は多くありましたが、十分に理解しきれないままプローブを当てていた場面もありました。今回の研修では、Dr. Maがエコーの内部構造、プローブの当て方、各臓器で注意すべきピットフォール、そして所見をどのように臨床判断につなげるかまで、非常に体系的に教えてくださいました。コンサルトを受けた患者さんに応じて、その場で100以上あると思われるレクチャーから関連するものを選んで指導してくださる教育スタイルは大変贅沢で、毎日が新しい学びに満ちていました。POCUSは単なる検査ではなく、患者背景を理解した医師が行うことにより、必要な情報を集め、次のステップ（Volume評価の場合は補液/利尿など）を決めるためのアセスメントができるのだと感じました。JVP、肺、心臓、IVC、腎臓、下肢静脈、関節など幅広い領域のPOCUSを経験させていただきました。POCUSはGeneralistにとって親和性の高い領域であり、自分もさらに学びを深めたいという気持ちが強まりました。

私は2026年7月からMount Sinai Morningside/Westで内科研修を開始する予定です。今回のACPおよびCalgaryでの研修は、自分がどのような内科医を目指したいのかを再確認する機会となりました。米国での研修では、臨床能力はもちろん、POCUS、身体診察、医学教育、そしてGeneralistとしての幅広い視点をさらに深めていきたいと考えています。そして将来的には、米国で得た学びを日本に還元し、日本の総合内科の発展に貢献してまいります。

最後になりますが、このような貴重な機会を与えてくださったACP日本支部長の矢野晴美先生をはじめ、ACP日本支部の先生方、Rising Star Scholarship Programに関わってくださった皆様、Calgaryで温かく迎えてくださったDr. Irene Ma先生とPOCUS teamの皆様、そして日々支えてくださっている職場の先生方・同僚の皆様に、心より感謝申し上げます。今回の経験を今後の臨床、教育、そして国際的なキャリア形成につなげていけるよう、引き続き努力してまいります。

I was truly honored to be selected as an awardee of the 2026 Rising Star Scholarship Program, which gave me the invaluable opportunity to attend the ACP Internal Medicine Meeting 2026 in San Francisco, as well as to participate in Point-of-Care Ultrasound (POCUS) training in Calgary, Canada.

＜Pre-course: April 14–15＞
For the pre-course, I attended two sessions: “Nutrition in Practice: Evidence-Based Strategies for the Internal Medicine Physician” and “Cardiology for the Internal Medicine Physician 2026: ‘The Key Points.’” Both courses were full-day lectures from 8 a.m. to 5 p.m. The Nutrition course covered a wide range of topics, including culinary medicine, supplements, malnutrition, dietary approaches for obesity and diabetes, enteral and parenteral nutrition, and micronutrient deficiencies. The course reminded me of the importance of providing patients with accurate, evidence-based nutrition information in everyday clinical practice, especially in this era when patients are surrounded by an overwhelming amount of health-related information. The Cardiology course allowed me to review and update my knowledge on topics that internists frequently encounter in daily practice, such as acute coronary syndrome, atrial fibrillation, heart failure, preoperative cardiovascular evaluation, cardiac imaging, valvular heart disease, hypertension, and ECG abnormalities. In both courses, it was notable that the speakers consistently referred to guideline recommendations, randomized controlled trials, and other forms of evidence to support their teaching.

＜ACP Internal Medicine Meeting 2026: April 16–18＞
During the main ACP meeting, I was impressed by the large number of sessions related to Artificial Intelligence. OpenEvidence was introduced as having achieved a perfect score on the USMLE, which made me realize how difficult it will be for physicians to compete with AI in terms of knowledge alone. At the same time, there was also discussion that many people still prefer to be cared for by human physicians, which made me reflect on the true value of physicians. The sessions explored the future role of AI in medicine from various perspectives, including its potential to reduce physician burnout through improved efficiency and the development of AI tools designed for patients.

A particularly memorable moment was learning directly from Dr. André Mansoor, the author of Frameworks for Internal Medicine, about the examination of jugular venous pressure at a hands-on physical examination session. He was very surprised when I shared that, at St. Luke’s International Hospital, one of my mentors recommends using the same wooden bell stethoscope as Dr. Mansoor, which I actually use in daily practice. His teaching was excellent, and I was also reminded that resources such as Physical Diagnosis PDX and Stanford Medicine 25 provide many valuable videos on physical examination. I hope to continue learning from these resources and further refine my bedside examination skills.

Beyond the sessions, I had many meaningful encounters throughout the ACP meeting. On several occasions, physicians sitting next to me during sessions casually started conversations, which naturally led to exchanging business cards. This made me realize that conferences are not only places to listen to lectures, but also valuable opportunities to meet physicians with shared interests and to build relationships that may shape one’s future career. Being able to place myself in such an environment through the Rising Star Scholarship Program was deeply encouraging as I continue to pursue learning in international settings.

＜University of Calgary POCUS Training: April 20–24＞
Following the ACP meeting, I had the privilege of joining the POCUS team led by Dr. Irene Ma at Foothills Medical Centre in Calgary. Although I had used ultrasound frequently in my daily clinical practice, there had been many occasions when I placed the probe without fully understanding the underlying principles or confidently interpreting the findings. During this training, Dr. Ma taught us in a highly structured way, beginning with the basic physics of ultrasound machines, probe handling, organ-specific pitfalls, and how to integrate ultrasound findings into clinical decision-making. Depending on the patients referred to the POCUS team, she would select relevant lectures from what seemed to be more than 100 teaching materials and teach us on the spot. Every day was filled with new learning. Through this experience, I came to understand that POCUS is not merely a diagnostic test. Rather, when performed by a physician who understands the patient’s clinical background, it becomes a powerful bedside tool for gathering essential information and guiding the next step in management. During the training, I had the opportunity to experience a wide range of POCUS applications, including JVP, lung, cardiac, IVC, renal, lower-extremity venous, and joint ultrasound. I felt that POCUS is highly compatible with generalist practice, which further motivated me to continue deepening my skills in this field.

I will begin my internal medicine residency at Mount Sinai Morningside/West in July 2026. Together, ACP and the POCUS training gave me an important opportunity to reflect on the kind of internist I hope to become. During my residency in the United States, I hope to further develop not only my clinical skills, but also my abilities in POCUS, physical examination, medical education, and broad generalist thinking. In the future, I hope to bring what I learn in the United States back to Japan and contribute to the development of general internal medicine.

Finally, I would like to express my deepest gratitude to Dr. Harumi Gomi, all members of the ACP Japan Chapter, Dr. Irene Ma and the entire POCUS team in Calgary, and my supervisors and colleagues who support me every day. I will continue to work hard so that this experience will shape my future clinical practice, teaching, and international career development.

この度、2026年度Rising Star Scholarship ProgramのAwardeesに選出いただき、サンフランシスコで開催されたACP Internal Medicine Meeting 2026およびUniversity of CalgaryのPOCUS teamへのobservershipという計2週間の海外研修に参加させていただきました。Generalistとしてのキャリアを考える上で大変貴重な機会となりましたので、以下にご報告いたします。

＜ACP Internal Medicine Meeting 2026: 4/14-18＞
PrecourseではPsychiatryとGeriatricsのセッションを受講し、本会期では緩和ケア・臨床推論・医学教育など幅広いセッションに参加しました。特に印象的だったのは参加者の質問への積極性で、講演後にはマイクの前に長蛇の列ができるのが常でした。疑問をそのままぶつける率直さは、自身の学びにも演者への敬意の表明にも繋がると感じ、日本でも見習いたい文化だと思いました。講義の合間には普段お話しできないような日本の先生方との交流の機会も多くいただき、大変楽しく、またありがたい時間となりました。

＜University of Calgary POCUS team: 4/20-24＞
Irene Ma先生率いるPOCUS teamを見学しました。最も印象に残ったのは、解剖を深く理解しているからこそ、プローブの動かし方を論理的に説明でき、Rock・Fan・Slide・Pivotといった操作をMa先生が丁寧に解説してくださったことです。質問すれば即座にテイラーメイドのレクチャーが提供される極めて教育的な環境で、POCUSが総合内科と非常に親和性が高いことを改めて実感し、帰国後の診療に取り入れていきたいと決意しました。
大変刺激的な2週間でした。引き続き英語学習と日々の臨床に取り組んでゆく励みとなりました。このような貴重な機会をくださった矢野晴美先生をはじめとするJapan Chapterの先生方、相棒の丹治先生に深く感謝申し上げます。

I am honored to have been selected as one of the Awardees of the 2026 Rising Star Scholarship Program. As part of this award, I attended the ACP Internal Medicine Meeting 2026 in San Francisco and participated in an observership with the POCUS team at the University of Calgary. These two weeks abroad proved to be an invaluable experience as I consider my career as a generalist. I would like to share my report below.

＜ACP Internal Medicine Meeting 2026: April 14-18＞
I attended the Precourse sessions on Psychiatry and Geriatrics, and during the main meeting I joined a wide range of sessions including palliative care, clinical reasoning, and medical education. What struck me most was the audience’s eagerness to ask questions: after each lecture, a long line invariably formed at the microphone. Their candor in voicing questions directly felt both like a means of personal learning and a sign of respect for the speaker, and it is a culture I would love to see take root in Japan. Between sessions, I also had many opportunities to speak with Japanese physicians whom I would not normally have a chance to meet, which made the time both deeply enjoyable and truly meaningful.

＜University of Calgary POCUS team: April 20-24＞
I observed the POCUS team led by Dr. Irene Ma. What impressed me most was that, thanks to a deep understanding of anatomy, the team could logically explain how probe movements translate into specific images, with Dr. Ma carefully walking us through each maneuver such as Rock, Fan, Slide, and Pivot. Any question prompted an immediate, tailor-made mini-lecture; it was an extraordinarily educational environment. The experience reaffirmed how well POCUS aligns with general internal medicine, and I returned home determined to incorporate it into my own daily practice.

These two weeks were truly inspiring, and have given me fresh motivation to continue both my English study and my daily clinical work. I would like to express my deepest gratitude to Dr. Harumi Gomi and the other members of the ACP Japan Chapter for this precious opportunity, and to my companion Dr. Tanji.

この度、Doctor’s Dilemmaに日本代表として出場し、ニューオリンズで世界の若手医師たちと知識を競い合ってきました。英語の早押し形式という非常に緊張感のある舞台で、結果は惜しくも初戦敗退となりましたが、一問一答に全力で挑んだ時間は、とても刺激的で、かけがえのない経験となりました。

世界の優秀な仲間たちと、医学という共通言語を通じて正々堂々と戦えたことは、何ものにも代えがたい財産です。自分の力不足を痛感する場面が多々ありましたが、それ以上に、医師という仕事の奥深さや尊さ、そして学び続けることの底知れない楽しさを、改めて実感しました。学会全体も、内科のあらゆる分野を網羅した充実のセッションばかりで、多くの学びを得ることができました。またニューオリンズは、歴史と芸術、音楽の息づく街であり、ジャズバーやミュージアムなど街そのものにも魅了されました。わずか4日間の滞在ではありましたが、心身ともに実り豊かな時間を過ごすことができました。

この出会いと挑戦の記憶を心の灯として、患者さんに寄り添う診療を大切にしながら、理想を追い求める姿勢を決して忘れず、歩んでいきたいと思います。

I had the great honor of representing Japan in the Doctor’s Dilemma at the American College of Physicians, held in New Orleans. There, I had the opportunity to compete with brilliant young physicians from around the world in a fast-paced, high-pressure quiz entirely in English. Although we were unfortunately eliminated in the first round, every moment spent answering each question with full concentration was deeply stimulating and truly unforgettable.

What left the greatest impression on me was the chance to engage in fair and spirited competition with peers from across the globe, united by the common language of medicine. While there were many moments in which I became acutely aware of my own limitations, the experience also reminded me of the profound depth and dignity of being a physician, and the endless joy that comes from continuing to learn throughout one’s career.

Beyond the competition, the conference itself was filled with enriching sessions that covered nearly every field of internal medicine. I gained valuable insights that I am confident will shape my future clinical practice. The city of New Orleans, with its rich culture, vibrant jazz bars, and world-class museums, added another layer of inspiration to the experience. Though my stay lasted only four days, it was a time full of intellectual and emotional fulfillment.

I will carry the memory of this encounter and challenge as a guiding light in my journey ahead—remaining close to my patients, practicing with sincerity, and never losing sight of the ideals I strive to uphold as a physician.

獨協医科大学病院総合診療科　佐藤由隆

今回Doctor’s Dilemma に参加し，英語で医学を学ぶことや刺激を受け続けることの重要性を感じました。
最も海外との差を感じたのはリスニングの瞬発力でした。Dilemmaでは出題者によって問題が読み上げられた後，スクリーンに英文の問題が表示されその後早押しクイズ勝負となります。アメリカのレジデントは読み上げられた時点ですでに早押しの用意をしていますが，私は読み上げられただけでは理解できず，問題文を文字で確認する分遅れます。

MKSAPなどの問題を解くだけでなく，英語でのアウトプット，ポッドキャストなどでの情報収集を学習に取り入れて対策していきたいと感じました。
また，決勝進出するレベルのレジデントの知識水準の高さにも驚きました。決勝では日本語ですらわからないと感じる問題が多く，それをミスなく回答していく様は圧巻でした。自分もさらに研鑽を積まなければならないと感じ，とても良い刺激を受けたと思います。

今回はDoctor’s Dilemmaに参加することができ貴重な経験を積むことが出来たと思います。一緒に参加してくれた同僚の宮崎先生や，日本チームとして壇上で戦った瀧宮先生，仲良先生をはじめACP日本支部の方々にもサポートいただきました。大変感謝しております。来年に向けて日々の臨床能力向上と知識の拡充に励んでいきたいと思います。

Participating in Doctor’s Dilemma this year made me acutely aware of the importance of continually engaging with medical education in English and maintaining motivation through stimulating experiences. The most significant gap I noticed ompared to international colleagues was my ability to rapidly comprehend spoken English. In Doctor’s Dilemma, questions are first read aloud by the moderator and then displayed on a screen, triggering a quick-response quiz competition. American esidents
were prepared to buzz in immediately after hearing the question, whereas I found myself delayed because I needed to isually confirm the written question to fully understand it.
To bridge this gap, I recognize the need to go beyond merely solving MKSAP-style problems. I intend to incorporate active English communication and listening practices, such as podcasts, into my learning routine. Additionally, I was impressed by the exceptional level of knowledge demonstrated by residents who advanced to the final rounds. The finals
featured numerous challenging questions, many of which I felt I would struggle to answer even in Japanese, yet the inalists answered them flawlessly. Witnessing this high standard inspired me greatly, underscoring the necessity for continued self-improvement.

この度は大変光栄なことに、アメリカのルイジアナ州ニューオリンズで開催されたInternal Medicine Meeting 2025のDoctor’s Dilemmaに日本チームの一員として出場させていただきました。今回の渡航に関して多大なるご支援を賜りましたACP日本支部の皆様、新年度の忙しい時期に快く送り出してくださった獨協医科大学病院総合診療科の皆様に心より感謝申し上げます。

Doctor’s Dilemmaはレジデントを対象とした医学知識を競うコンペティション企画で、もともとは所属元の上級医に勧められて参加した前年度の国内大会が始まりでした。内容としてはクイズ大会に近く、当初は単にレクリエーションと認識して参加しておりましたが、その過程でこれは極めて有効な学習の機会であることに気づいていきました。

第一に、日本・アメリカいずれの大会でも非常に問題の質が高く、大会に参加するだけでも最新のエビデンスに基づいた医学知識をインプットする機会となります。特に競技形式ですので、正解したときの嬉しさ、間違えたときの悔しさというエピソード記憶に基づいて入力された内容はなかなか忘れません。

次に、チーム戦であるため大会までの間にチームや所属元の仲間と切磋琢磨できる点があります。実際、今回の国際大会に向けてはMKSAP 20を仲間とともに1ヶ月程度で解ききり、最終的にはほとんど正答できる状態で臨むことができました。この学習速度は自分ひとりではなし得なかった成果だと思っています。

そして最後に、そのようにして参加させていただいた国際学会は、Doctor’s Dilemmaだけでなく各分野最前線のセクションや、学会終了後のエキスパートの方々との懇親会など、単に知識の習得に留まらない非常に貴重な経験となりました。レジデントにとっては学会へエントリーするきっかけとしても大変有用な企画でした。

改めてこのような貴重な機会をいただきましたことに厚く御礼申し上げます。ここで得た経験を糧に日々の臨床でも研鑽を積んでいきたいと考えています。

I was deeply honored to participate in the Doctor’s Dilemma at the Internal Medicine Meeting 2025, held in New Orleans, Louisiana, as a member of the Japan team. I would like to express my heartfelt gratitude to the ACP Japan Chapter for their generous support in making this trip possible, and to the Department of Diagnostic and Generalist Medicine at Dokkyo Medical University Hospital for kindly allowing me to attend during the busy start of the new academic year.

Doctor’s Dilemma is a medical knowledge competition designed for residents. My journey began with a recommendation from my senior physician to participate in the national preliminary round last year. The format is similar to a quiz bowl, and while I initially viewed it as a recreational activity, I soon realized how effective it was as a learning opportunity.

First and foremost, the quality of the questions—both in the Japanese and U.S. competitions—was exceptionally high. Simply participating in the event provided an excellent opportunity to absorb up-to-date, evidence-based medical knowledge. Because of the competitive format, the emotional highs of answering correctly and the frustration of incorrect responses created strong episodic memories, making the content easier to retain.

Secondly, as a team competition, the preparation process allowed us to challenge and motivate each other. In fact, leading up to the international round, our team worked together to complete all of MKSAP 20 in about a month. By the time of the competition, we were able to answer most questions correctly—a level of preparation I could not have achieved alone.

Finally, attending the international conference was an invaluable experience far beyond the competition itself. In addition to Doctor’s Dilemma, I had the chance to attend cutting-edge sessions in various fields and engage with experts during social events after the conference. It proved to be a meaningful gateway into the world of international conferences for residents like me.

I would once again like to extend my sincere appreciation for this extraordinary opportunity. I am committed to applying what I have learned through this experience to my daily clinical practice and continuing to grow as a physician.

2025年4月にニューオリンズで開催されたIMM 2025 の参加報告

2025年4月10日から12日、アメリカ・ニューオリンズで開催された米国内科学会（ACP）年次総会2025に参加し、ポスター発表を行いました。

今回の参加は、米国内科学会日本支部総会2024で発表したIgA血管炎の症例報告により黒川賞を受賞し、Chapter Winnerに選出いただいたことがきっかけです。支部からの温かいご支援により、初めて海外の学会に参加する貴重な機会をいただき、心より感謝しております。

会場となったErnest N. Morial Convention Centerは規模も活気も圧倒的で、世界各国から集まった医師や医学生たちの熱意に刺激を受けました。
ポスター発表では、IgA血管炎の診断・治療経過について報告し、米国の先生方から多くの質問や意見をいただく中で、異なる医療アプローチを肌で感じることができました。

また、ポスターセッションの合間には、近くで発表していたアメリカのレジデントたちと交流し、互いの研修環境やキャリア観について語り合う機会にも恵まれました。この経験は、自分にとって非常に大きな励みとなりました。

今回得た学びを今後の臨床・研究活動に活かし、さらに成長していきたいと思います。貴重な機会をいただいたすべての皆様に、心より御礼申し上げます。

Report on Attending ACP 2025 in New Orleans

From April 10 to 12, 2025, I went to New Orleans, USA, to join a big medical meeting called ACP 2025. I gave a poster presentation there.

I joined because I won the Kurokawa Award after sharing a report about a disease called IgA vasculitis at the Japan Chapter meeting of ACP in 2024. Thanks to warm support from the chapter, I was able to join an international meeting for the first time. I am very thankful.

The place of the meeting, Ernest N. Morial Convention Center, was very big and full of energy. Doctors and medical students came from many countries. I was very excited and learned a lot from them.

In my poster presentation, I talked about how we found and treated the IgA vasculitis case. Many doctors from the U.S. asked me good questions. I could learn how medical care is different in other countries.

During free time in the poster session, I also talked with American resident doctors near my poster. We shared stories about our training and future dreams. That was a very happy and helpful time for me.

I want to use what I learned here in my future study and work. I thank everyone who gave me this great chance.
