この度、Doctor’s Dilemmaに日本代表として出場し、ニューオリンズで世界の若手医師たちと知識を競い合ってきました。英語の早押し形式という非常に緊張感のある舞台で、結果は惜しくも初戦敗退となりましたが、一問一答に全力で挑んだ時間は、とても刺激的で、かけがえのない経験となりました。

世界の優秀な仲間たちと、医学という共通言語を通じて正々堂々と戦えたことは、何ものにも代えがたい財産です。自分の力不足を痛感する場面が多々ありましたが、それ以上に、医師という仕事の奥深さや尊さ、そして学び続けることの底知れない楽しさを、改めて実感しました。学会全体も、内科のあらゆる分野を網羅した充実のセッションばかりで、多くの学びを得ることができました。またニューオリンズは、歴史と芸術、音楽の息づく街であり、ジャズバーやミュージアムなど街そのものにも魅了されました。わずか4日間の滞在ではありましたが、心身ともに実り豊かな時間を過ごすことができました。

この出会いと挑戦の記憶を心の灯として、患者さんに寄り添う診療を大切にしながら、理想を追い求める姿勢を決して忘れず、歩んでいきたいと思います。

I had the great honor of representing Japan in the Doctor’s Dilemma at the American College of Physicians, held in New Orleans. There, I had the opportunity to compete with brilliant young physicians from around the world in a fast-paced, high-pressure quiz entirely in English. Although we were unfortunately eliminated in the first round, every moment spent answering each question with full concentration was deeply stimulating and truly unforgettable.

What left the greatest impression on me was the chance to engage in fair and spirited competition with peers from across the globe, united by the common language of medicine. While there were many moments in which I became acutely aware of my own limitations, the experience also reminded me of the profound depth and dignity of being a physician, and the endless joy that comes from continuing to learn throughout one’s career.

Beyond the competition, the conference itself was filled with enriching sessions that covered nearly every field of internal medicine. I gained valuable insights that I am confident will shape my future clinical practice. The city of New Orleans, with its rich culture, vibrant jazz bars, and world-class museums, added another layer of inspiration to the experience. Though my stay lasted only four days, it was a time full of intellectual and emotional fulfillment.

I will carry the memory of this encounter and challenge as a guiding light in my journey ahead—remaining close to my patients, practicing with sincerity, and never losing sight of the ideals I strive to uphold as a physician.

今回Doctor’s Dilemma に参加し，英語で医学を学ぶことや刺激を受け続けることの重要性を感じました。
最も海外との差を感じたのはリスニングの瞬発力でした。Dilemmaでは出題者によって問題が読み上げられた後，スクリーンに英文の問題が表示されその後早押しクイズ勝負となります。アメリカのレジデントは読み上げられた時点ですでに早押しの用意をしていますが，私は読み上げられただけでは理解できず，問題文を文字で確認する分遅れます。

MKSAPなどの問題を解くだけでなく，英語でのアウトプット，ポッドキャストなどでの情報収集を学習に取り入れて対策していきたいと感じました。
また，決勝進出するレベルのレジデントの知識水準の高さにも驚きました。決勝では日本語ですらわからないと感じる問題が多く，それをミスなく回答していく様は圧巻でした。自分もさらに研鑽を積まなければならないと感じ，とても良い刺激を受けたと思います。

今回はDoctor’s Dilemmaに参加することができ貴重な経験を積むことが出来たと思います。一緒に参加してくれた同僚の宮崎先生や，日本チームとして壇上で戦った瀧宮先生，仲良先生をはじめACP日本支部の方々にもサポートいただきました。大変感謝しております。来年に向けて日々の臨床能力向上と知識の拡充に励んでいきたいと思います。

Participating in Doctor’s Dilemma this year made me acutely aware of the importance of continually engaging with medical education in English and maintaining motivation through stimulating experiences. The most significant gap I noticed ompared to international colleagues was my ability to rapidly comprehend spoken English. In Doctor’s Dilemma, questions are first read aloud by the moderator and then displayed on a screen, triggering a quick-response quiz competition. American esidents
were prepared to buzz in immediately after hearing the question, whereas I found myself delayed because I needed to isually confirm the written question to fully understand it.
To bridge this gap, I recognize the need to go beyond merely solving MKSAP-style problems. I intend to incorporate active English communication and listening practices, such as podcasts, into my learning routine. Additionally, I was impressed by the exceptional level of knowledge demonstrated by residents who advanced to the final rounds. The finals
featured numerous challenging questions, many of which I felt I would struggle to answer even in Japanese, yet the inalists answered them flawlessly. Witnessing this high standard inspired me greatly, underscoring the necessity for continued self-improvement.

この度は大変光栄なことに、アメリカのルイジアナ州ニューオリンズで開催されたInternal Medicine Meeting 2025のDoctor’s Dilemmaに日本チームの一員として出場させていただきました。今回の渡航に関して多大なるご支援を賜りましたACP日本支部の皆様、新年度の忙しい時期に快く送り出してくださった獨協医科大学病院総合診療科の皆様に心より感謝申し上げます。

Doctor’s Dilemmaはレジデントを対象とした医学知識を競うコンペティション企画で、もともとは所属元の上級医に勧められて参加した前年度の国内大会が始まりでした。内容としてはクイズ大会に近く、当初は単にレクリエーションと認識して参加しておりましたが、その過程でこれは極めて有効な学習の機会であることに気づいていきました。

第一に、日本・アメリカいずれの大会でも非常に問題の質が高く、大会に参加するだけでも最新のエビデンスに基づいた医学知識をインプットする機会となります。特に競技形式ですので、正解したときの嬉しさ、間違えたときの悔しさというエピソード記憶に基づいて入力された内容はなかなか忘れません。

次に、チーム戦であるため大会までの間にチームや所属元の仲間と切磋琢磨できる点があります。実際、今回の国際大会に向けてはMKSAP 20を仲間とともに1ヶ月程度で解ききり、最終的にはほとんど正答できる状態で臨むことができました。この学習速度は自分ひとりではなし得なかった成果だと思っています。

そして最後に、そのようにして参加させていただいた国際学会は、Doctor’s Dilemmaだけでなく各分野最前線のセクションや、学会終了後のエキスパートの方々との懇親会など、単に知識の習得に留まらない非常に貴重な経験となりました。レジデントにとっては学会へエントリーするきっかけとしても大変有用な企画でした。

改めてこのような貴重な機会をいただきましたことに厚く御礼申し上げます。ここで得た経験を糧に日々の臨床でも研鑽を積んでいきたいと考えています。

I was deeply honored to participate in the Doctor’s Dilemma at the Internal Medicine Meeting 2025, held in New Orleans, Louisiana, as a member of the Japan team. I would like to express my heartfelt gratitude to the ACP Japan Chapter for their generous support in making this trip possible, and to the Department of Diagnostic and Generalist Medicine at Dokkyo Medical University Hospital for kindly allowing me to attend during the busy start of the new academic year.

Doctor’s Dilemma is a medical knowledge competition designed for residents. My journey began with a recommendation from my senior physician to participate in the national preliminary round last year. The format is similar to a quiz bowl, and while I initially viewed it as a recreational activity, I soon realized how effective it was as a learning opportunity.

First and foremost, the quality of the questions—both in the Japanese and U.S. competitions—was exceptionally high. Simply participating in the event provided an excellent opportunity to absorb up-to-date, evidence-based medical knowledge. Because of the competitive format, the emotional highs of answering correctly and the frustration of incorrect responses created strong episodic memories, making the content easier to retain.

Secondly, as a team competition, the preparation process allowed us to challenge and motivate each other. In fact, leading up to the international round, our team worked together to complete all of MKSAP 20 in about a month. By the time of the competition, we were able to answer most questions correctly—a level of preparation I could not have achieved alone.

Finally, attending the international conference was an invaluable experience far beyond the competition itself. In addition to Doctor’s Dilemma, I had the chance to attend cutting-edge sessions in various fields and engage with experts during social events after the conference. It proved to be a meaningful gateway into the world of international conferences for residents like me.

I would once again like to extend my sincere appreciation for this extraordinary opportunity. I am committed to applying what I have learned through this experience to my daily clinical practice and continuing to grow as a physician.

2025年4月にニューオリンズで開催されたIMM 2025 の参加報告

東京ベイ•浦安市川医療センター/伊東市民病院　内科専攻医　佐藤駿一

2025年4月10日から12日、アメリカ・ニューオリンズで開催された米国内科学会（ACP）年次総会2025に参加し、ポスター発表を行いました。

今回の参加は、米国内科学会日本支部総会2024で発表したIgA血管炎の症例報告により黒川賞を受賞し、Chapter Winnerに選出いただいたことがきっかけです。支部からの温かいご支援により、初めて海外の学会に参加する貴重な機会をいただき、心より感謝しております。

会場となったErnest N. Morial Convention Centerは規模も活気も圧倒的で、世界各国から集まった医師や医学生たちの熱意に刺激を受けました。
ポスター発表では、IgA血管炎の診断・治療経過について報告し、米国の先生方から多くの質問や意見をいただく中で、異なる医療アプローチを肌で感じることができました。

また、ポスターセッションの合間には、近くで発表していたアメリカのレジデントたちと交流し、互いの研修環境やキャリア観について語り合う機会にも恵まれました。この経験は、自分にとって非常に大きな励みとなりました。

今回得た学びを今後の臨床・研究活動に活かし、さらに成長していきたいと思います。貴重な機会をいただいたすべての皆様に、心より御礼申し上げます。

Report on Attending ACP 2025 in New Orleans

From April 10 to 12, 2025, I went to New Orleans, USA, to join a big medical meeting called ACP 2025. I gave a poster presentation there.

I joined because I won the Kurokawa Award after sharing a report about a disease called IgA vasculitis at the Japan Chapter meeting of ACP in 2024. Thanks to warm support from the chapter, I was able to join an international meeting for the first time. I am very thankful.

The place of the meeting, Ernest N. Morial Convention Center, was very big and full of energy. Doctors and medical students came from many countries. I was very excited and learned a lot from them.

In my poster presentation, I talked about how we found and treated the IgA vasculitis case. Many doctors from the U.S. asked me good questions. I could learn how medical care is different in other countries.

During free time in the poster session, I also talked with American resident doctors near my poster. We shared stories about our training and future dreams. That was a very happy and helpful time for me.

I want to use what I learned here in my future study and work. I thank everyone who gave me this great chance.

この度、2025年度Rising Star ScholarshipのAwardeesに選出頂くという光栄に預かり、Generalistとしてのキャリアを考える上での貴重な機会として、New Orleansで開催されたACP Internal Medicine Meeting 2025と、CalgaryでのPOCUS teamへのobservershipという、計2週間の海外研修に参加させて頂きました。

＜Precourse: 4/1-2＞
Precourseは「Diabetes for Internal Medicine Physicians」と「Geriatrics and Palliative Medicine for Internal Medicine Physicians」の2講座を受講しました。朝8時から夕方5時まで続く怒涛のレクチャーを英語で受け続けるのはタフで心が折れそうでしたが、事前配布された200ページに及ぶpdf資料が非常に充実しており、それを頼りに何とかついていくことができました。いずれも各領域の最新のエビデンスを網羅したReview形式で、若手よりもベテランの先生方が多く参加されていたのが印象的でした。
Diabetesのセッションでは、アメリカにおける課題といえる肥満症に関連したGIP/GLP-1受容体作動薬の話題が多く取り上げられ、また持続血糖測定(CGM)がOTCで広く活用されていることに驚きました。
Geriatric Medicineのセッションでは、「5Ms」に基づいたアプローチを学びました。フレイルに対する薬物治療の可能性が提示される一方で、認知症周辺症状に対して日本で頻用されているような漢方薬は当然紹介されず、高齢者診療において日本からエビデンスを発信する余地を感じました。
講師の誕生日にサプライズでケーキが用意されているような遊び心がありつつも、参加者が活発に質問し熱心に学んでいる様子に刺激を受けました。

＜Internal Medicine Meeting 2025: 4/3-5＞
Precourseより大幅に参加者が増え、世界最大の内科学会の名に恥じない規模に圧倒されました。日本では考えられない大きさの大会場で行われたオープニングセレモニーで、Tuckson先生がトランプ政権下でのワクチン問題や誤情報の拡散について「Physicianはmedical misinformationの最前線にいる」と力強く訴え、会場全体が割れんばかりのスタンディングオベーションで応えた場面に強く感銘を受けました。その後も数々のセッションに参加し、緩和ケア・がん検診・臨床推論・診断エラーなどの日常診療に直結するテーマはもちろん、関心のある医学教育、さらにはアメリカならではのカンナビノイドに関する話題など、非常に幅広いトピックに触れることができました。また広大な展示ブースも印象的で、アメリカ海軍/陸軍のコーナーがあったり、日本発の薬剤が大々的に宣伝されている場面もあったり等、興味深く拝見しました。
また、懇親会では普段なかなかお会いできないような先生方と交流させていただき、診療へのモチベーションが大いに高まりました。Centor scoreで著明なCentor先生、現職場上司の指導医であったMeyer先生にお会いできたことがハイライトです。
参加者の多くがGeneralistであったからか、学会全体に流れる空気感が心地よかったです。そのような環境は日本の現状とは隔世の感を受けましたが、日本のGeneralismを発展させるべく頑張ってゆく決意を新たにしました。

＜University of Calgary POCUS team: 4/7-11＞
Foothills Medical CentreでIrene Ma先生率いるPOCUS teamの見学をさせて頂きました。Ma先生＋レジデント4名の計5名から成るPOCUS専門のコンサルテーションチームで、相談のあった患者さんに関してルーチンで肺エコー/心エコー/IVC・JVP測定によるvolume評価を行ったり、エコーガイド下で腹腔/胸腔穿刺を行ったり、時には皮膚軟部組織を見たりと、全身のエコーを数多く経験させて頂きました。医師の働き方や医療環境の違いに驚かされることもありましたが、Ma先生が「Beautiful!」「Perfect!」など多彩な表現でレジデントを褒められ、隙間時間にすかさずシミュレーションやレクチャーをされる指導スタイルは、医学教育に関心がある私にとって多いに刺激になりました。また、自身が日常的にエコーを用いる機会が少ないことを反省し、POCUSが総合内科・家庭医療という自分の診療領域と非常に親和性が高いことを改めて実感しました。帰国後にPOCUSを日々の診療や教育にぜひ取り入れていこうと決意しました。
Ma先生やレジデントの皆さんの素晴らしいホスピタリティにも感激し、充実した楽しい1週間を過ごすことができました。

総じて、大変刺激的な2週間を過ごさせて頂きました。英語での議論についていくための自身の語学力不足を反省する一方で、臨床面に関しては日本での実践も必ずしも海外基準に劣っていないことも多いのではないかと感じ、英語学習や日々の臨床を引き続き頑張る励みになりました。
改めまして、このような貴重な機会をくださった矢野晴美先生をはじめとするJapan Chapterの先生方、年度初めという忙しい時期にもかかわらず2週間の不在を許してくださった職場の皆さま、そして2週間一緒に濃い時間を過ごさせて頂いた相棒の石原洋先生に、深く感謝申し上げます。本当にありがとうございました。

ACP Annual Conference Internal Medicine Meeting 2025

Rising Star Scholarship Program Awardee

I was truly honored to be selected as the awardees for the Rising Star Scholarship Program 2025. This provided me with a valuable opportunity to participate in two weeks of international training: attending the ACP Internal Medicine Meeting 2025 in New Orleans and joining an observership with the POCUS team in Calgary. This experience greatly influenced my outlook as a generalist.

＜Precourse: April 1-2＞
Prior to the main meeting, I attended two precourse sessions: “Diabetes for Internal Medicine Physicians” and “Geriatrics and Palliative Medicine for Internal Medicine Physicians.” Attending full-day lectures in English from 8 a.m. to 5 p.m. was highly demanding, but the well-prepared 200-page PDF materials provided in advance enabled me to keep up with the sessions. Each course offered a comprehensive review of up-to-date evidence in the field, and many experienced physicians were in attendance. In the Diabetes session focused on topics such as GIP/GLP-1 receptor agonists related to obesity, and the widespread over-the-counter use of continuous glucose monitoring (CGM) devices in the U.S., which was surprising to me. In the Geriatrics session, I learned an approach based on the “5Ms.” While new pharmacologic approaches to frailty were introduced, there was no mention of Kampo medicine, which is often used in Japan to treat behavioral and psychological symptoms of dementia (BPSD). This highlighted for me the potential to contribute Japan-generated evidence to the global geriatric field. Despite the intense schedule, the friendly atmosphere, including a birthday celebration for one of the instructors and active audience participation, made the sessions engaging and inspiring.

＜Internal Medicine Meeting 2025: April 3-5＞
Compared to the precourse, the number of participants increased significantly, and the scale of the event was impressive. At the opening ceremony, Dr. Tuckson addressed the challenges of vaccine misinformation under the Trump administration, emphasizing that physicians are on the front lines of medical misinformation. His speech received a standing ovation, which was deeply moving. Over the next few days, I attended a wide range of sessions, covering topics directly applicable to everyday clinical practice—such as palliative care, cancer screening, clinical reasoning, and diagnostic error—as well as subjects aligned with my personal interests, including medical education and uniquely American themes such as cannabinoid use in clinical care. The large exhibition hall featured booths from the U.S. military and promotional displays for Japanese-developed drugs, which were interesting to observe. At the networking events, I had the rare opportunity to connect with distinguished physicians, which greatly boosted my motivation. A particular highlight was meeting Dr. Centor—well-known for the Centor score—as well as Dr. Meyer, a former mentor of my current supervisor.
The overall atmosphere was warm and collegial, likely due to the many generalists in attendance. It was a refreshing contrast to the environment in Japan and strengthened my commitment to advancing generalism back home.

＜University of Calgary POCUS team: April 7-11＞
Following the ACP meeting, I observed the POCUS consultation team at Foothills Medical Centre in Calgary, led by Dr. Irene Ma. The team consisted of Dr. Ma and four residents and conducted routine assessments, including lung and cardiac ultrasound, IVC/JVP evaluation, and ultrasound-guided procedures. I was able to gain hands-on experience with a wide range of scans. I was surprised by the differences in medical practice and working culture, but also deeply impressed by Dr. Ma’s positive teaching style—using varied praise such as “Beautiful!” and “Perfect!” to encourage residents, and seamlessly integrating simulations and mini-lectures into the day’s schedule. At the same time, This experience also made me reflect on my own limited use of ultrasound and helped me realize the strong relevance of POCUS in general internal and family medicine. I am now determined to incorporate POCUS into my daily practice and teaching in Japan. I was also deeply touched by the warm hospitality of Dr. Ma and the entire POCUS team, which made for a thoroughly enriching and enjoyable week.

Overall, these two weeks were an eye-opening and highly stimulating experience. While I recognized the need to improve my English for international discussions, I also felt confident that many aspects of clinical practice in Japan are on par with global standards. This realization encouraged me to continue improving both my language skills and clinical practice.

Finally, I sincerely thank Dr. Harumi Yano and the ACP Japan Chapter for this opportunity, my colleagues at Fukuchiyama City Hospital for supporting my absence during a busy time of year, and Dr. Yo Ishihara, who shared this enriching experience with me. Thank you very much.
